If you use cfindex to index a directory and if you do NOT specify extensions as an attribute, nothing is indexed. This seems pointless. If I don't specify extensions, it should index everything.

As per the doc, is the extension is not specified, by default we use these extensions - htm, html, cfm, cfml, dbm and dbml to index. Are you saying that none of these extensions are getting picked and indexed?
